• The signatory is authorised to sign on behalf of the organisation
• The information contained is, to the best of their knowledge, correct
• The organisation agrees to provide all necessary evidence at the approval visit in order to assess suitability against IoSCM requirements
• The organisation agrees to allow access to external quality assurance staff acting on behalf of IoSCM to ensure requirements are being met in relation to the quality of the delivery and certification process

IoSCM will hold and use the information for any purpose deemed relevant to the resale of qualifications
